The Continuity of Care Maturity Model (CCMM) is an eight stage (0-7) model that demonstrates the evolution of communication in healthcare. The CCMM goes beyond the EMRAM maturity model to help organisations assess their whole system. Learn how this model recognises the importance of continuity of care between multiple providers in a number of care settings. 

 

Addressing the UK Digital Healthcare Challenge | 1:50 - 2:30 p.m.

The long-term plan outlined by the National Health Service has set out an ambitious strategy for improving the quality of healthcare delivery in the UK over the next decade. Learn how both policy and technology advances are enabling the University Hospital Southampton NHS Foundation Trust in developing award-winning innovations with other NHS Trusts to deliver digital healthcare capabilities more quickly and cost-effectively. This session will outline how a successful interoperable PHR was implemented to eliminate data silos, allow information to flow freely between organisations and care teams and is improving the delivery of patient care.

Looking to the Future: Healthcare Technology and Hyland | 3:15 - 4 p.m.
Francis Tella, Product Manager, Hyland

This presentation will offer insight into current and future healthcare technology development efforts at Hyland.  We will also discuss technology trends and direction, how they align with the NHS Long Term Plan, and how Hyland is assisting its customers to be well positioned to respond to the changing healthcare landscape.

Q: How is this event different than the Hyland Summit | EMEA?
The Hyland Healthcare Forum has an agenda tailored specifically for healthcare organisations. The Hyland Summit | EMEA offers a rich agenda relevant to all Hyland customers, not just those in healthcare. 

Q: What is the cost to attend the forum? 
There is no registration fee to attend the event. Attendees are responsible for their hotel and travel, if needed. We will provide a complimentary lunch and cocktail reception.

Q: When is the last day I can register for the forum? 
We will allow for on-site registration but encourage you to register in advance as space is limited and registrations are accepted on a first-to-register basis.

Q: If I received an invitation to attend, am I automatically registered?
